How Google AI Overviews Are Changing Clinic Advertising: A Guide for Marketers

More advertising spend does not necessarily produce a matching increase in clicks. For example, if spending rises 9% while clicks rise 4%, cost per click rises approximately 4.8%, using 1.09 divided by 1.04. The five-percentage-point gap between the two growth rates is not itself the CPC calculation.

The Korean article attributed those figures to a Q4 2025 benchmark; its underlying dataset was not independently recovered here. A separately checked WordStream report found higher cost per lead in 13 of 23 industries and a reported 2025 CPC benchmark of US$5.26. Its figures are technically medians from 16,446 US campaigns running April 2024–March 2025, rather than Korean clinic cost forecasts.

Google's AI Overviews are one change worth examining when clicks or inquiries fall. Auction competition, search demand, tracking and the landing-page experience also need review. The presence of an AI summary alone does not establish why a clinic's advertising performance changed.

This guide covers four developments and practical questions for clinic marketers. The Korean original was published April 23, 2026; this English edition checks sources and product information as of October 31, 2026.

1. Search-result layouts are changing, but ads are not always pushed below the summary

AI can add another element to the results page

A traditional result might contain these elements, in a layout that varies by query:

Search ads
A local map pack, when relevant
Organic results

A result with an AI Overview may combine:

Search ads above or below the AI Overview
An AI summary with supporting links
Ads within AI content only where eligible
Other results, which may include maps and organic links

Google's current guidance explicitly allows standard ads above or below an AI Overview. Neither a fixed number of cited clinics nor a fixed number of advertising slots applies to every search. Some people may find enough information in the summary; others continue to the source or another result.

What the historical click-through study measured

Seer's September 2025 update, published November 4, analyzed 3,119 informational or educational search terms across 42 client organizations, with 25.1 million organic and 1.1 million paid impressions:

  • For the study's AI-Overview query group, paid CTR moved from 19.70% in June 2024 to 6.34% in September 2025, a reported decline of approximately 68%.
  • The same group's paid CTR reached 3.26% in July 2025. This is not a measurement that every commercial clinic query fell from 11% to 3% in that month.

These historical associations are a reason to inspect a clinic's own queries. The dataset should not be relabeled as a study of Korean LASIK, implant or botulinum-toxin advertising. Seer's April 24, 2026 follow-up also reported that the earlier downward trend was leveling off or reversing in its newer data, so continued decline is not a certainty.

2. Ads can appear within AI experiences in eligible markets

Google introduced ads within AI Overviews in the US in 2024 and has expanded availability. Check the current product guidance before using a past rollout announcement to plan a clinic campaign.

Eligibility depends on commercial relevance, the query and response context, the auction, available campaign formats, markets, languages and applicable advertising policies.

What this means for a clinic

As checked for this edition, Google's current guide lists English availability within AI Overviews and AI Mode in specified markets; South Korea is not in that published list. Placement above or below an Overview has a broader scope. Healthcare advertising also has its own policies and restrictions. A generic product announcement is not confirmation that a particular clinic can advertise inside an AI response.

Seer's older study reported 35% higher organic CTR and 91% higher paid CTR for cited brands in its Q3 comparison. These are associations in a particular dataset, not proof that earning a citation raises a clinic's results by 90% at the same budget.

Organic citations and advertising eligibility are separate

Google says buying ads does not influence organic AI answers, and organic citation does not determine ad-auction eligibility. A clinic missing from an AI answer is not automatically unable to compete through ads. Measure the actual campaign and appointment journey instead of assuming a competitor's citation guarantees a revenue advantage.

3. Korean medical-advertising requirements need their own review

Changes in search products and legal requirements are different questions. A clinic marketing plan needs to account for both.

Review the medium and content, including the exceptions

Current Medical Service Act Article 57 and Enforcement Decree Article 24 identify covered media and limited-information exceptions. The relevant social-media provisions include a 2018 amendment; they should not be described as a new blanket December 2024 rule for every public post.

The 366 advertisements cited in the original article came from monitoring on December 11, 2023–February 10, 2024, announced March 11, 2024. They were not a new December 2024–March 2025 sweep of unreviewed ads. The release counted 506 potentially unlawful content items, including 183 testimonial items. Its printed 31.7% conflicts with that denominator; the reported 135/26.7% and 126/24.9% use the content-item basis.

Criminal limits and administrative standards

The 2024 announcement described maximum penalties of one year or KRW 10 million for false or exaggerated medical advertising, and three years or KRW 30 million for unlawful patient inducement or brokerage. It also cited institutional or professional suspension standards. These are not automatic cumulative sanctions for every advertisement; the applicable rule and facts matter.

Plan testing and revisions around actual approval requirements

Determine whether each proposed item is medical advertising, whether the medium is covered, and whether an exception applies. Keep the approved version and check the relevant procedure before changing it. This is more precise than assuming that all A/B testing is prohibited or that every outside sentence needs advance review.

4. Fewer clicks and a higher conversion rate can coexist

WordStream's 2025 benchmark reported improved conversion rates in many industries. That does not demonstrate that AI Overviews caused the improvement. A smaller number of visitors can include a different mix of people, while targeting, tracking, offers and website changes also affect the rate.

What should clinic marketers compare?

A narrow planning approachA more useful review
Increase spend to restore trafficReview qualified inquiries and confirmed appointments alongside spend, content and search visibility.
Treat informational and appointment queries alikeSegment intent and evaluate actual performance before changing bids or budgets.
Prioritize faster promotional publishingMaintain useful, substantiated content and the appropriate clinical and advertising review.
Assume patients equal spend divided by a unit costTrack actual outcomes; multiplying an AI citation rate by advertising efficiency is not a validated patient-acquisition formula.

5. Four practical steps for clinic marketers

1. Separate query intent, then assess the evidence

  • Informational questions: prepare useful explanations about risks, options and recovery. Evaluate any paid activity on its own results rather than declaring all informational advertising ineffective.
  • Location, brand and appointment questions: make accurate information and the next step easy to find. Check policy, cost and confirmed inquiry quality before deciding how much to advertise.

2. Replace unsupported slogans with supportable information

An unsupported “Gangnam's No. 1 LASIK clinic” claim needs review. So does a statement about an internal procedure count or recovery outcome: it needs real records, a defined measure, relevant limitations and the applicable advertising assessment.

The original illustration's “5,000 procedures” and “two-day recovery” are not verified clinic results supplied for this article. Do not copy them into an advertisement. Clear data presentation does not automatically obtain an AI citation or medical-advertising approval.

3. Review useful content as a possible landing page

A relevant guide can help visitors who arrive from an ad. Check the entire page, its claims, clinical information, contact route and any required approval. A content page's previous review does not automatically approve a new advertisement, destination or edited version under Google's rules or Korean law.

4. Keep local profiles and the website accurate

Check Google Business Profile and relevant Korean listings against the clinic's actual information. A reliable profile and useful website can support discovery and decision-making. They do not ensure that every AI answer uses those sources in the same order.

Discuss your clinic's search, content and advertising review with BlinkAd. We can help organize the evidence around costs, observed visibility and patient inquiries, with clinical and regulatory questions directed to the appropriate review. Any campaign change still needs its own eligibility and policy checks.
